Traditionally, a target serum urate level of below 6 mg/dL has been widely recommended for all stages of gout. However, emerging research suggests that a more personalized approach, adjusting the target based on the stage of treatment, may be beneficial.

According to Nicola Dalbeth, MBChB, MD, FRACP, FRSNZ, head of the department of medicine at the University of Auckland, New Zealand, different serum urate targets may be more effective at different phases of treatment. This means the ideal level for dissolving existing crystals may differ from the level needed to maintain long-term remission and prevent new crystal formation.

While specific recommendations may vary depending on individual factors and the advice of your rheumatologist, the general idea is that a more aggressive urate-lowering strategy may be warranted initially to rapidly dissolve crystals and alleviate symptoms. This might involve aiming for a target below 5 mg/dL in the early stages of ULT. Once symptoms are well-controlled and tophi are resolving, the target might be relaxed slightly to maintain a level below 6 mg/dL.
